Why we run it that way

Because examination has degraded into ritual almost everywhere. Trainees perform it, document it, and wait for imaging — which works in a well-resourced hospital and fails completely where the echo is next week, which describes a great many of our learners’ hospitals.

Honest about reliability

The text is unusual in stating where signs are unreliable rather than presenting all of them as discriminating. Modules follow suit: some cases are constructed so that the examination genuinely cannot settle the question, and the correct answer is to say so and investigate.
