The First Hour modules are our only ones that run in real time. The scenario clock does not pause while a learner thinks, and the simulated patient deteriorates on schedule regardless. Learners find their first session genuinely unpleasant, which is the closest a remote course can get to the actual experience.

This text supplies the sequences those scenarios mark against.

Why a protocol text rather than a reference

Under time pressure a clinician does not need to understand the pathophysiology; they need the next action. Each of the sixteen chapters is therefore an ordered sequence — establish this, give that, prepare the other, and escalate at this point if there is no response. Our debriefs score against those sequences directly.

Acute coronary presentation and reperfusion. Arrest and post-resuscitation. Unstable arrhythmia. Pulmonary oedema. Shock, including the compensated patient who is not. Aortic emergency. Tamponade. Hypertensive crisis. Acute valve failure.

Prerequisite

The general cardiology track or equivalent clinical experience. Learners without it may take the reading and the didactic modules but are not enrolled into timed scenarios, where the point is decision speed rather than knowledge acquisition.
