The module about patients who say no
Our Behaviour Change block spends a session on the patient who has declined structured exercise, will not join a gym, and has no intention of changing that. Learners are asked what they offer next, and most have nothing.

Why it works as a recommendation
Twenty minutes outdoors has no cost, no equipment, no membership and no self-consciousness attached. It is one of very few recommendations that patients who refuse everything else will still attempt, which makes its modest evidence base more valuable in practice than a stronger intervention nobody adopts.
Taught with the evidence stated honestly
The module is explicit that much of the research is observational and the effect sizes modest. Learners are asked to phrase the recommendation to a patient without overclaiming — a harder exercise than it appears, and directly transferable to every other lifestyle conversation they have.
