Starting an antibiotic is easy and most learners do it correctly. The Stewardship block is built around the harder decision — day three, the patient is better, the cultures are unhelpful, and someone has to decide whether to narrow, switch to oral, or stop. Nobody wants to be the person who stopped too early, so nothing changes and the course runs for ten days.

Case format

Cases run forward in days rather than hours. Learners make a decision on day one, then see the day-three culture, then decide again — and the module reveals what the patient’s actual course was. It is one of the few blocks where the correct answer is frequently to do less.

The cardiac infection modules

Given their own block because they are where cardiology learners are least confident. Infective endocarditis with prolonged regimens and surgical timing. Device infection, where extraction is the management. Sternal wound infection. Rheumatic fever prophylaxis.
